Spay & Neuter Contract

Permanent ownership of this dog is contingent upon your compliance with this Addendum.

Proof of the spay/neuter must be received by the breeder within fourteen (14) days after the surgery. In the event that the dog’s health does not allow this agreement to be honored, Golden Harvest Doodles, LLC must be provided with a statement from a Licensed Veterinarian stating that the dog is not yet in physical or emotional condition for surgery.

Based upon the Veterinarian’s evaluation, the purchaser must establish the earliest possible date for the procedure and notify the breeder in writing. All other conditions of this agreement will remain in effect.

It is the responsibility of the purchaser—not the veterinarian—to ensure that Golden Harvest Doodles, LLC receives verification that the surgery has been completed. Verification must include:

• Copy of medical records
• Receipt of procedure
• Veterinarian’s phone number for confirmation

Failure to comply with this agreement by the required date, unless otherwise agreed to in writing, will be considered a breach of contract. The purchaser will be required to return the dog to Golden Harvest Doodles, LLC and will not be entitled to a refund.

In the event of the puppy’s death prior to the required spay/neuter date, a written statement from a Licensed Veterinarian must be provided, including the cause of death.

Failure to provide proper documentation will result in a $1,000 fee, representing the difference between pet price and breeding price.

If ownership must be transferred before spay/neuter is completed, the purchaser agrees to notify Golden Harvest Doodles, LLC prior to placing the puppy in a new home.

The breeder retains the right of first return. The puppy must be returned to the breeder if the purchaser cannot keep the dog. The puppy may not be given away, sold, or transferred without breeder approval.

If the purchaser transfers ownership without notifying the breeder, they will be liable for the full breeding value of the dog, which is $1,000 more than the pet price.

Agreement Terms

By entering into this agreement, the purchaser agrees to have the dog spayed or neutered by one year (12 months) of age.

This agreement confirms that the dog will not be used for breeding, whether intentional or accidental.

If the dog is not spayed/neutered as agreed, the purchaser agrees to return the dog to the breeder with no refund.
